Submitting a Pcard Form

Pcard forms need to be filled out and saved locally before submitting the form for review.

• OBM Payment Card Forms

1. Select the form to open.

2. Fill out the required information.
3. Click the Save button.
   • Save the file locally to your computer.

Uploading the form

1. Fill in your Agency BU .
2. Select the Form Type you are submitting.
   • Use "CTRL" and "SHIFT" to select multiple form types.
3. Enter any special instructions in the Comments section.
4. Click the Choose File  button in the File Upload section.
5. Navigate and select the file that was saved above.
6. Click Open.
7. Click Submit Form.
